Before Your Appointment


What you need to know before you come in for your appointment.

  • Please fill out your online paperwork at least 2 days prior to your appointment to allow us to prepare for your exam and verify your insurance. You can click on the link here: ONLINE PAPERWORK, the passcode will be the last 4 of your social security number. 
  • Please arrive 15 minutes early. We need the extra time to be sure that you’re entirely done with paperwork and complete diagnostic testing before the exam begins.
  • Due to appointment demand, you may be charged a minimum $39 fee if you cancel or miss your appointment without 24-hour notice.
  • You must bring a valid driver’s license or a photo ID for identification purposes. Your ID will be scanned and put into your patient file for future reference, unless you request not to have this done.
  • Be sure to write a list of any prescription medications you’re currently taking, and don’t forget to list the dosage as well. This information will be very helpful for our doctors.
  • If you wear glasses of any kind – sunglasses, readers, computer glasses, driving glasses, etc. – please bring all of them to your exam.
  • If you wear contact lenses, please bring your lenses and names of any solutions you use. We will also need the contact lens prescription – the boxes that your contact lenses came in will work just as well.
  • Your insurance ID card is essential. Please bring anything you have pertaining to your insurance coverage, especially your medical and routine vision coverage.

We strongly encourage you to call your insurance plan and verify your benefits and eligibility before coming in to see us.

  • If a copay is due, or if your exam will not be billed to insurance, those charges must be paid at time of service.
  • If you do not want to be dilated, be sure to ask our front desk about “Optomap® ” screening. In certain cases, we may be able to inspect the back of your eye without the need for dilation. However, please note that while this method is quick, painless, and effective, it is not yet covered by insurance and there will be a $39 charge. Our doctors strongly recommend having this procedure done at least once to have a photographic record in your file to refer back to if needed later.
  • Please be sure that all cell phones and other electronic devices are turned off during your appointment.
